Plan for at least one day in Milano. The consultation is at the office of Dr. Guichet. You get a complete clinical examination and a thorough discussion with Dr. Guichet of the 'custom-made' preparation you will undergo according to your stature and proportions and of the indication. You will receive documents explaining how to succeed in your lengthening along with the procedure to follow, and you will discuss a possible operative period or date. A first evaluation of costs will be made, according to options you would like to get.
You may meet patients in the process of lengthening or thereafter. If you wish to meet with further patients, you will go later to the Isokinetic Rehab Center. If you booked exams, you can get X-rays, an isokinetic Cybex testing of your muscles, etc.
Pre-operative preparation will consist on a full preparation of your body and a psychological coaching to optimize results. This avoids the risks associated with a lack of preparation. You will be guided through all steps and benefit from the best current knowledge on how to prepare both physically and psychologically.
You can prepare in a gym close to your home. However, in a secondary step or initially, one options is to stay in Milano to prepare under the control of Dr. Guichet and his team. Alternate placs controlled by Dr. Guichet are in London and Marseille with highly performing professional teams. A preoperative gain of 30% of muscle force is a good basis for securing the result with a lengthening procedure.
You will enter the private hospital the day of surgery for optimal, personal and confidential care. You can have a family member with you, TV, telephone and Internet. Immediately after the operation, Dr. Guichet will initiate with you the postoperative procedure. On the first post-operative day, you will resume full weight bearing, walking and cycling in your room, sometimes earlier. Discharge is as early as the second day, but usually occurs on the third post-op day.
After surgery, you will be 5 to 10 mm taller (1/4 to 1/2 inch)! Full motion is resumed the day of the operation for easing clicks and preventing pain. 15 clicks per day make 1 mm of gain (1/24 inch). Clicks are performed every 8 hours, according to a custom program that Dr. Guichet will give you. A follow-up book will allow you to record ratchets and physical exercises explained with photographs. A personal trainer will can take care of you like a professional athlete to maximize your functional recovery, including sports, and nutritional advice. When needed, a physiotherapist and other professionals will take care of you. You will have regular follow-ups and visits 'tailored' to your needs with Dr. Guichet.
Major part of problems occur in the lengthening and healing phases, that is why staying in the Center set-up by Dr. Guichet during the lengthening period decreases all potential problems of a lengthening surgery and provides the optimal set-up for succeeding, even during the healing phase, through the guidance and recommandations provided during the lengthening phase by the Team of Dr. Guichet.
Bone healing usually takes 4 to 6 months. Daily life is gradually resumed. The sports to practice or to avoid will be explained. X-rays will be regularly performed.
It is generally performed as a day surgery procedure (in outpatient clinic), 18 months after insertion. Crutches are often recommended for 1 to 3 weeks. Efforts must be avoided for approximately 3 months. In general, sports including competitions are gradually and completely resumed after a lengthening.
Some added procedures are sometimes performed to treat associated problems related to the shortening/lengthening disease.
